We’re seeking a Care Management Facility Director to lead with compassion, strategy, and vision. In this critical role, you'll oversee care coordination, discharge planning, and social work operations—ensuring our patients receive the physical, emotional, and spiritual support they need to truly feel whole.
What You’ll Do:
🔹 Lead a dynamic team of Care Managers, Social Workers, and Assistants
🔹 Drive strategies to improve outcomes, reduce readmissions, and streamline transitions of care
🔹 Collaborate closely with hospital leaders (CNO, CMO, CFO)
🔹 Ensure compliance with CMS and best practices in care management
🔹 Mentor and grow the next generation of compassionate healthcare leaders
What You Bring:
✔ BSN or MSW (MSN/LCSW preferred)
✔ 5+ years of Care Management experience, 3+ in leadership
✔ Deep understanding of discharge planning, utilization review & patient advocacy
✔ A heart for service and a drive to lead with empathy
