Here’s what you’ll be doing:

• Support the case management team with administrative tasks, insurance and benefit verifications, data collection, and documentation to ensure smooth care coordination and reimbursement.
• Coordinate patient care services, including Durable Medical Equipment (DME), oxygen, home health services, transportation, and discharge planning across the full continuum of care.
• Act as a resource to staff, physicians, patients, families, and providers for discharge planning, community resources, financial counseling, MediCal applications, and authorizations.
• Manage data entry, maintain department schedules, compile appeal requests, handle medical record requests, and assist with EMTALA, transfer, and referral forms.
• Participate in meetings, special projects, and process improvement initiatives to support case management operations and enhance patient care outcomes.

Here’s what you’ll have:

• Supports case management processes, including insurance and benefit verifications, outcome management, discharge planning, and acting as a resource for staff, patients, and providers regarding community resources.
• Coordinates Continuum of Care services such as physical therapy, home health, Durable Medical Equipment (DME), oxygen, transfers to other facilities, and referrals to lower levels of care.
